The following forms 1 & 2 must be provided for all NEW players who have not played for Davis Hot Shots in the past.

2)  PLAYER-PARENT_CONTRACT.pdf    (new players to Davis Hot Shots only)

 

3)  ALL PLAYERS:   Current AAU Card with Exp Date 8/31/2026 or later  https://aausports.org 

    (Click on Join AAU top left corner)
